New Patient Info
Welcome to our practice! We are thrilled that you've selected us to help care for your child's dental needs! Our goal is to help make your entry into our practice as smooth as possible. This section of our website is designed to help with this process.
To begin, you'll need to complete some paperwork which will offer us some information on your family's contact information, your child's medical history, communication and privacy forms, etc. We are pleased to tell you that we are a paperless, chartless office - we have created a secure section of our website that allows you to fill out these forms and then have them immediately sent to us. The good news is that this will save you some time from having to do this when you arrive at our office! Please click the link below and you will be able to select the forms for completion:

Insurance Friendly Environment
Fact 1 - NO INSURANCE PAYS 100% OF ALL PROCEDURES
Dental insurance is meant to be an aid in receiving dental care. Many patients think that their insurance pays 90%-100% of all dental fees. This is not true! Most plans only pay between 50%-80% of the average total fee. Some pay more, some pay less. The percentage paid is usually determined by how much you or your employer has paid for coverage or the type of contract your employer has set up with the insurance company. For non-preventive dental treatment, we will ask you to pay 20% of the treatment at the time the service is rendered.
Fact 2 - BENEFITS ARE NOT DETERMINED BY OUR OFFICE
You may have noticed that sometimes your dental insurer reimburses you or the dentist at a lower rate than the dentists actual fee. Frequently, insurance companies state that the reimbursement was reduced because your dentists fee exceeds the usual, customary, or reasonable fee ("UCR") used by the company. A statement such as this gives the impression that any fee greater than the amount paid by the insurance company is unreasonable or well above what most dentists in the area charge for a certain service. This can be very misleading and simply is not accurate. Insurance companies set their own schedules and each company uses a different set of fees they consider “reasonable.”
We accept cash, personal checks, and most major credit cards. There is a $35.00 service charge for all returned checks. If it becomes necessary to forward your account to small claims court, you will be responsible for the fee charged by the courts for costs of collections in addition to the amount of the bill. In addition, should an account age over 90 days, the practice reserves the right to assess a $25.00 Late Charge on the account.
MOST IMPORTANTLY, please keep us informed of any insurance changes such as policy name, insurance company address, or a change of employment.
We are committed to providing your child with the highest quality dental care using only the best material available in dentistry today in a child-friendly enviornment. Our financial policy is intended ot facilitate excellent service to you while minimizing our administrative costs. As a condition of treatment by our office, please understand that fees must be paid at the time the service is performed and, as part of our paperless billing initiative, a current credit card must be kept on file to address any outstanding patient balances including those resulting fom insurance claim settlements. This credit card will be autobilled once the insurance company has settled your claim. Please feel free to keep a Flex Card on file - or a credit card that can help you build up mileage or points :)
